Services & Scope of Practice
Florida Scope of Practice: Under HB 607 (eff. 7/1/2020), autonomous APRNs may independently practice primary care including family medicine, general pediatrics, general internal medicine, and related health services โ without a physician supervisory protocol. โ ๏ธ Important Scope Limitations: Autonomous NP practice in Florida is currently limited to primary care (family medicine, general pediatrics, general internal medicine). Specialty services (dermatology, cardiology, etc.) still require physician collaborative agreements. CRNAs and specialty APRNs are not included in autonomous practice provisions. If the NP holds psychiatric certification, HB 883/HB 301 (2025โ2026 session) may expand autonomous psychiatric practice โ monitor legislative progress.
Licensing, Credentials & Compliance
- Active FL APRN License: Unencumbered, no disciplinary action within prior 5 years (per ยง456.072 / ยง464.018 F.S.)
- 3,000 Clinical Hours: Under physician protocol within the past 5 years (FL or out-of-state with protocol)
- 45 CE Hours: In pharmacology AND 45 CE hours in differential diagnosis within past 5 years (or equivalent graduate coursework)
- National Certification: AANP, ANCC, or other recognized certifying body โ must be current
- DEA Registration: Required for prescribing controlled substances (Schedule IIโV)
- NPI Numbers: Type 1 (individual) and Type 2 (organizational) โ required for insurance billing
- Biennial Renewal: Autonomous registration renews with APRN license cycle
- Active FL RN License: Via NCLEX-RN examination or endorsement (MOBILE Act: 2 years active practice)
- IV Therapy Certification: INS (Infusion Nurses Society) CRNI preferred; or documented IV competency training
- BLS/ACLS Certification: Current American Heart Association certifications
- Standing Orders: Must operate under written protocols/standing orders from the NP (or collaborating physician)
- CE Requirements: Florida requires 24 CE hours per biennium for RN renewal
- No Special Mobile Credential: Standard FL RN license is sufficient โ no separate "mobile nursing" license exists
- Florida LLC/PLLC Registration: Sunbiz.org โ Professional LLC if NP-owned practice
- AHCA Health Care Clinic License: Required for mobile clinical operations (Agency for Health Care Administration)
- CLIA Waiver Certificate: Required if performing any CLIA-waived testing (rapid strep, glucose, flu, COVID, urinalysis)
- Lee County Local Business Tax Receipt: Required occupational license for Bonita Springs base
- EIN (Federal Tax ID): IRS โ required for business banking, payroll, insurance billing
- HIPAA Compliance Program: Written policies, BAAs, encrypted EHR, staff training documentation
- Biomedical Waste Registration: FL DEP โ required for sharps, blood-contaminated waste disposal
- Pharmacy Relationship: Compounding pharmacy agreement for custom IV formulations (503B pharmacy)
- CAQH ProView: Universal credentialing application โ required by virtually all payers
- Medicare PECOS Enrollment: NP enrolls as rendering/billing provider โ required for any Medicare patients
- Medicaid Provider Enrollment: Florida Medicaid (AHCA) โ optional based on target demographics
- Commercial Payers: BCBS, Aetna, UHC, Cigna, Humana โ individual credentialing per plan
- Note: Most mobile IV/wellness services are self-pay; insurance credentialing is primarily for NP primary care services
- Taxonomy Codes: NP: 363L00000X (Family) or 363LP0200X (Pediatrics); RN: 163W00000X
Reality Check: The majority of mobile IV therapy patients are self-pay. Medicare/Medicaid and most commercial insurers do not cover wellness IV infusions. Insurance credentialing is strategic for primary care/sick visit services to expand revenue beyond cash-pay wellness.
